1. Form should be printed on the Company’s letterhead.

2. Date the form.

3. Either signed by the Company’s director or an authorized person together with an authorization letter signed by a director.

4. Provide a photocopy of the (a) Company’s Business Registration Certificate; (b) Certificate of Incorporation and (c) latest Annual Return.

5. Kindly pass the completed form to one of the Hong Kong Chambers of Commerce (such as Hong Kong General Chamber of Commerce; Federation of Hong Kong Industries, etc.) for endorsement before delivering to our Consulate together with a photocopy of the form.

6. HK$152for each seal (cash only).

We need two working day to deliver the endorsed document.
